When breast ptosis (sagging) occurs, most hospitals recommend a mastopexy (breast lift) as the first corrective method.
While mastopexy is a common and effective method for resolving sagging issues, it has the drawback of leaving visible scars on the front of the breast.
At JW Plastic Surgery, we perform mastopexy only when absolutely necessary.
For patients whose breasts are relatively small and whose sagging is not severe, we take a more conservative and refined approach, correcting the issue through augmentation techniques or internal lifting methods such as adjusting the degree of muscle dissection with the dual-plane technique or applying an internal fixation lift instead of performing a traditional breast lift.

Crescent-Shaped Incision Method

Refined Dual-Plane Technique
When a breast implant is placed beneath the pectoralis major muscle, the muscle wraps around the implant, creating a more natural and softer feel. The dual-plane technique involves partially releasing the lower part of the pectoralis muscle to reduce muscle tension, resulting in a more natural breast shape and texture. Additionally, as the dissected muscle exerts an upward pull, this technique helps to lift the breast tissue naturally, making it particularly effective for sagging breasts. Different types of dual-plane dissection techniques are applied depending on the degree of breast sagging.

While most hospitals classify the traditional dual-plane technique into only three types (Type 1, 2, and 3), the JW Dual-Plane Technique, developed through extensive experience, is categorized into Type 0.5 to 3.0 (in 0.1 increments).
This highly detailed classification allows for precise correction of sagging and breast asymmetry.

Effective Use of the Inframammary Fold Incision
In general, sagging breasts have a deep inframammary fold and reduced skin tension in that area.
Therefore, using an inframammary fold incision is often the better option than incisions made in other locations.

Internal silicone inside the implant
There is no need to wear a surgical drain, and arm movement is easy even immediately after surgery,
so there is less discomfort in daily activities, such as housework, childcare, or work.
Even when the inframammary fold incision is the more suitable option,
surgery can still be performed through an armpit incision if the patient prefers.

Breast size asymmetry can be relatively easily corrected by using implants of different sizes for each breast or by transferring different amounts of fat to each breast when fat grafting.

Wearing a Special Surgical Bra
Even if both breasts are made as symmetrical as possible during breast augmentation surgery, the position of the implants may shift over time depending on lifestyle habits or the natural characteristics of the breast tissue.
For example, right-handed people tend to use their right arm and pectoral muscles more frequently, and since the heart is located on the left side, the height and angle of the rib cage differ slightly between sides.
The surgical bra helps maintain proper implant positioning, correcting any imbalance and providing consistent support to prevent displacement.
In addition, after augmentation surgery, the inframammary fold can sometimes appear less defined, creating an unnatural look. The surgical bra helps redefine the fold and expand the lower breast area, enhancing the overall natural appearance of the breasts.
