Breast augmentation techniques are generally divided into implant augmentation and fat grafting. At JW Plastic Surgery Center, we offer Combination Breast Augmentation (Implants + Fat Grafting), an advanced technique designed to combine the advantages of both methods.
Combination breast augmentation enhances both breast volume and natural softness by placing a breast implant while simultaneously performing precise fat grafting. This approach allows for improved contour, a more natural feel, and refined shaping.
During implant-based breast augmentation, an HD endoscopic system is utilized to precisely and safely create the implant pocket. The use of high-definition endoscopy allows for enhanced visualization during surgery, improving accuracy and safety.
Many patients visit the clinic with an approximate implant size in mind after conducting their own research. However, since every individual has a unique body structure and breast anatomy, the same implant size can produce very different results from person to person.
The appropriate implant size must be carefully determined by considering the patient’s overall physique – including height, weight, and chest circumference – as well as the existing breast anatomy, such as skin elasticity, breast tissue volume, and the amount of muscle and fat.
Breasts that are excessively large relative to the body may not achieve balanced aesthetic proportions and may also place unnecessary strain on the spine or affect the natural texture of the breast.
The most precise decision is often made during surgery, when the surgeon can directly evaluate the anatomy and make adjustments accordingly.
For this reason, a full range of implant sizes must always be available during the procedure, ensuring that the most appropriate implant size can be selected to achieve the best possible outcome.
Breast augmentation incisions can generally be categorized into four types: axillary (underarm), periareolar (around the areola), inframammary fold (under the breast), and transumbilical (navel).
For the axillary incision, JW Plastic Surgery utilizes an HD endoscopic system, allowing the surgeon to perform the procedure with enhanced precision and visibility. This technique minimizes damage to surrounding tissues and nerves, resulting in reduced pain and a smoother recovery. The incision is placed within the deepest natural crease of the underarm, making the scar discreet and barely noticeable.
The inframammary fold incision, located beneath the breast, avoids the underarm area entirely. As a result, patients typically experience less discomfort in the arm and axillary region after surgery compared to the axillary approach. This method is particularly effective for patients with a well-defined inframammary fold, as the scar can be naturally concealed under the breast. It is also advantageous when correcting breast ptosis (sagging) in combination with augmentation.
During breast augmentation, a breast implant is inserted through a small incision. As this procedure involves placing a foreign material into the body, it requires the highest level of surgical precision and sterility to minimize risks such as infection or implant damage.

In breast augmentation, inserting an implant through a small incision requires advanced surgical skill and extensive experience. At JW Breast & Body, Dr. Sul is able to perform the procedure through a minimal incision while maintaining safety and precision.
To minimize visible scarring, the dermal layer is carefully closed using absorbable sutures, which naturally dissolve over time. The surface of the skin is then sealed with medical-grade skin adhesives such as Dermabond or specialized adhesive strips (Steri-Strip), eliminating the need for external stitches and stitch removal.

During breast augmentation, tissue separation and pocket creation naturally involves bleeding and tissue trauma. The surgery is performed under general anesthesia to ensure patient comfort. Following the procedure, pain management medication is provided during the recovery period at the clinic to support a smooth postoperative experience.

In certain cases, anti-nausea and pain control medications may be continuously administered for up to 24 hours through a PCA (Patient-Controlled Analgesia) device, even after discharge. This method helps reduce pain and nausea more effectively than oral medication, allowing patients to recover more comfortably.
Even when implants are placed as symmetrically as possible during breast augmentation surgery, their position may slightly change over time due to individual body structure and daily lifestyle habits. For example, in right-handed individuals, the right arm is typically used more frequently, which can influence muscle activity around the chest. Additionally, the left side of the chest naturally accommodates the heart, which may cause subtle anatomical differences in the rib structure compared to the right side. Because of these natural asymmetries, achieving perfect balance between both sides of the body can be challenging.
A postoperative correction bra helps support the breasts during the early healing period and assists in maintaining proper implant positioning. It helps guide the breasts into a balanced and stable position while the tissues are adapting after surgery.
When surgery is performed under general anesthesia, preoperative medical evaluations such as blood tests, electrocardiograms (EKG), and chest X-rays are required to ensure patient safety.
